Boston led by as many as 18 points late in the first half before Miami steadily clawed back into contention on aggressive play by LeBron James and Dwyane Wade and smothering defence.
A three-point shot by James in the final minute of regulation pushed the contest into overtime, but the league Most Valuable Player fouled out of the game with two minutes left in the extra session.
Captain Paul Pierce top scored for Boston with 23 points and Kevin Garnett added 17 and 14 rebounds. James led the Heat with 29 points.
